Some of the most well received gifts I have given over the years are often theme gifts. I wrapped two childrens books about puddles and rain with an umbrella and duck rain coat to give to my sister-in-law and loved the reaction from everyone. And my niece looked adorable in it!
It's always good to think ahead too. Babies grow out of clothing and simple toys quickly so consider giving a gift that is for a 6-12 month-old or even 2-3 year-old. I remember receiving a sleeper for my daughter in size 2 and thinking "that's huge" and I couldn't imagine her growing in to it -- but she did -- and I was greatful to have a new few items around as she grew.
